DOH-BAY PROMOTES 2022 COMMUNITY HEALTH ASSESSMENT SURVEY

Panama City, Florida— The Florida Department of Health in Bay County, in partnership with the Bay County Community Health Assessment (CHA) Steering Committee, invites you to participate in a health needs research study from the community. The survey asks questions about the health of the community and once the data is collected, the health needs are prioritized to focus on the most important needs of the communities. This information can help develop a community health improvement plan by justifying how and where resources should be allocated.

“You may be wondering why do we need a health assessment survey? Your participation in the survey will help identify gaps in our community’s health and nutrition resources, services and needs. Housing, schools, workplaces, transportation, parks and recreation, and health care are integral parts of our habitat. Therefore, closing the gaps in our built environment and public health will help create a healthier community,” said Sandon S. Speedling, MSH, CPM, CPH, Florida Department of Health in Bay County, Administrator and Health Officer .

We want to know how you perceive our community; your opinions are important. The link to the survey will go live on May 9 and close on May 31. The questionnaire will take approximately 5-10 minutes to complete and all responses are confidential. Please visit: https://www.surveymonkey.com/r/SKCSW3H or scan the QR Code below:
